I, consent and authorize Eagle Mountain International Church, Kenneth Copeland Ministries, its nominees, designees, successors, representatives, assigns, or affiliated organizations (herein referred to as “Producer”) to use the filming, videotaping, recording, and/or photography of my likeness, image and/or voice, in which may be included in whole, in part, or in composite in conjunction with other images and/or voices through any media worldwide for art, advertising, charity, commerce, business, trade, or any other lawful purpose whatsoever. I waive any right, title, or interest of any kind or nature whatsoever to ownership, copyrights, licensing of film, video files, prints, recordings, or posts. I also release, discharge, and agree to hold harmless the “Producer” from any liability and hold harmless the “Producer” from and against any and all loss, personal injury, death, costs, damages, or claims of any nature, including, but not limited to attorney’s fees, arising from growing out of or concerning the above production in any way. I certify that I am at least eighteen years of age, unless otherwise noted below.
